I just wanted to share that I found the link under my account...when I clicked the upgrade link, i had to get to the point of viewing all of the phones, then sort by price low to high.
Once I had done that, it was there and I ordered.
I am 11 months into the $99 / year 1GB plan

While speaking to them, the rep confirmed that this is only for accounts that are 9+ months old. This is locked to Boost for the initial 12 months.
NB: No active service is needed for unlocking after a year, even though they say otherwise in their unlocking policy. However, it needs to be activated first and it's IMEI should show up in your account to be eligible for unlocking.
I've been trying, but grossly unsuccessful so far!
