Most T-Mobile stores don't like accepting trade ins on the spot since something can go wrong and they take the blame
It will send to the email of the primary holder's account.
You may go back to costco and ask them to send out the email again. Also check you spam folder.
the screen is cracked and 85% is unable to display, but i was able to screen shot the IMEI and expand it in gallery to move it around to get the IMEI.
