Can I also just submit the trade-in in any T-mobile store as opposed to mailing it back via USPS? Not sure if the gift card gets cancelled if I do in-store trade-in submission as opposed to mailing it using the assigned shipping label.

I figure there are 3 possibilities:
1. We all got pranked by OP, no Costco gift card program
2. Costco has $75/$150 gift card program for this trade-in, for all plans
3. Costco has a SELECTIVE gift card program (i.e.: only for specific plans/conditions)
I think more people been told #2, so let's hope for the best

My guess is that with the break up of the Costco stores with different providers, they're paying $75 to find the one with t mobile in it
Which wasn't the one closest to me.. those were ATT and Verizon

Print the label from your T-Mobile account, pack it up and take it to the post office.
I'd be sure to get a receipt when you drop it off.
