I called the rebate center and was told the reason of deny is that I had multiple submission. I never did that, and I can only see one submission online which was the one I did. Next day, when I checked my mail box, the $75 Costco card was sitting there. The card was sent to my address, not the phone shipped to address.

So, here is my theory: sometime in April, T-Mobile changed their rebate system so that customer no long need to submit rebates (for Costco location?). At that time, T-Mobile sent bunch text to those customers who did the promo in January and voided their manual submission and re-created the same submission automatically for them. T-Mobile for sure knows the upgrade location as Costco agents need to receive their commissions. Rebates will send to T-Mobile account holder's address by default. As long as you do your upgrade at Costco T-Mobile booth, T-Mobile don't need to know if you have Costco membership or not, assuming you need have the membership in order to access the booth inside. I don't know T-Mobile even care about that or not.

If the imei doesn't match, the trade in will be rejected and you lose the promotion credits. There is no way t-mobile agents can change the imei after order is placed unless you cancel and re-order.

You have 2 options:
1. cancel and return, and re-order with correct imei.

2. contact t-force, and explain to them and have them put a note on your account confirming it's t-mobile's error and you should be eligible for the promotion even if the trade in is rejected due to wrong imei. After the trade-in is rejected and promotion drops off your account, contact t-force again and if they are nice enough, they will just manually pay off/terminate the EIP, which is actually better because you don't have to wait for 24 months for the credits.

The previous promo on January will not help here. Back then they were membership wireless. They have since made new deals with Costco as they now provide their own service instead of third party.
Had family done one since the T-Mobile take over. Indeed the promo is done automatically online. Only difference is that theirs show as promo "received" on the T-Mobile promotion page in just a few days. This Moto edge promotion does not show on the promotion page even after a week has passed.
It may be that it might work after 90 days but as of right now, who knows.

I am NOT SURE if this has changed recently. I remember reading this back when the iPhone XR and iPhone 11 was released and Costco was doing crazy trade in deals.
at one point an iPhone 6 was eligible for a big $730 or $830 rebate ( I forget the exact amount )
The guys processing the trade-ins won't even look at what is hand written on the receipt.
They just scan the tracking number/RMA barcode, and load the IMEI information saved in the system.
Hand written IMEI on the receipt won't do anything and you still have to contact t-mobile to fix the issue of wrong IMEI.

Thank you...just did that and they added promo and account note...and told me send old devices and let them reject it and they will adjust it and give me monthly credits to make it free

So do we need to activate/put a sim card in the phone or just wait for 90 days and let the phone sit in the box? Wanted to keep this as a backup.

i was told by phone agents that phone needs to be active on the line for $75 promo card.

yeah. they didn't seem to indicate for how long, but based on what they mentioned and previous anecdotal experiences mentioned here, i do think you have to use the phone for atleast few days (for it to register on their portal) which would probably trigger the gift card.

7/25 - Did my order through Costco kiosk.
7/26 - Received new phone in mail.
7/27 - Mailed old phone to T-Mobile.
8/8 - Notified via email/text trade-in was accepted
