It looks not as much as a glitch but as something intentional for the order/sale to go through. - If your birthday is in August and you had it saved in your Samsung profile, you should have gotten or will get an e-mail with an extra $25 off discount which can stack with the above codes

In the meantime, if you want to place a new order now, create a new account so that you can use the reserve code again, get a new unique code from CS, and place the order.
You'll only be missing the points, but use a referral code from your old account to your new account for another 5% off. And don't forget the 12% CB.

Are they just charging $99 for the pre-activation? Any issue if I go with the first trade-in and activate once the phone arrives?

You can have it on multiple orders, but you must re-add it via the Deals/Explore tab. If it's no longer there, then you're SOL.
If I list the offer, it doesn't have the Samsung one listed
