Also - any guidance on finding which Costcos have Verizon? When I search online and in the Denver area, all I get are TMo and AT&T

MVNO also usually don't have activation fee and monthly tax & fees are included in the advertised price, not in addition to like the 3 major carriers. Those fees add up to $100~150 a year and in 3 years, 1/3 of your $1000 discount is gone.
Of course this is Slickdeals so there will be more of the 5%ers here promoting this deal. But you really need to crunch the numbers to see if it makes sense for you. 3 years is a lot of time for Verizon to make changes to the plan you are signing up for and since the iPhone is locked until you paid it off you won't be jumping carrier anytime soon if fee go up
