Correct me if I'm wrong, shouldn't an impact screwdriver rotate with each hit?

…. I don't see any rotation in the illustration or read about rotation in the description.

It does rotate but not in a complete circle, just partly. When you hit the driver with a hammer the forward motion hits an angled ramp, it imparts both twisting and forward motion. So it presses down on the screw and turns it at the same time. The twisting motion is small, and only lasts as long as the hammer strike but it is enough to get the thing lose. Usually people whack it with a hammer from softly to increasing strength if it does not come lose, till it does.

You actually raise a reasonable question - I did not DV you. That said, if you look at the fourth graphic (closely) you'll see that the PH1 or PH2 head upon a good strike to the metal handle striking cap, what it does is sinks the specially shaped head *into* the screw. That extra bit of purchase in the screw, is supposed to allow you to extract stripped screws.

I bought one of the PH1's and PH2's but haven't used them, yet. So, I can't comment on how effective they are with very stubborn stripped/seized screws. So, this would not replace a normal impact driver (with a bit holder), but it is a nice option to have in your tool chest or tool bag.
