So Costco one is combination of Extend2Fit + 4Ever DLX (I believe sticker price is $379)
Ideally, if your kid has crossed age of 2 or 2.5 yr old and if he/she already started forward facing, then you don't need Extend to Fit.
Research shows, it is more safe to keep the baby rear facing and some countries in Europe make it mandatory to keep the baby rear facing till they turn 4 year old (read from another slickdeals comment) !!

I'm waiting to see if they'll lower the price on black Friday for the Costco one. In warehouse is currently 260 which is the lowest price I've seen on it so far. I have 4ever dlxs and the grandparents have slimfits, so far they've been great. The extend's weight limit from 40lb to 50lb for rear facing seems like a good upgrade for me so that's why I'm going for it.
I've never had a rotating one. They do look really convenient if your kid is really fussy about getting strapped in, if that's important to you. My kids and I aren't too picky so what we have works.
