Shouldn't be approved already at this time? Anyway I can change request the gift card to mail to a different address? I'll probably give a call to costco t-mobile to check since they are not picking up at the moment.
But you can give them a call, tmobile promotion #, and they can investigate the status of the gift card, they can either resend or just credit it to your tmobile account if the gift card doesn't get used at all after verified

Called customer service and the rep said that it could happen if the device had water damage or was locked. But since there wasn't any comment about it in the notes he just re-applied the promo.
