I can't tell if this includes toner. And I'm wondering if this means it's not as reliable of a refurb as the ones Brother sells themselves:

I'm pretty sure that Walmart buys these refurbs directly from Brother, and they are widely reported to arrive in brand new condition. Starter toner is included, good for hundreds of pages. This is a steal of a price for a printer of this caliber.

I am out of the printer market for about 10 years or so.

If I have a pdf/jpg file on my iPhone, how do I print it out to this printer without a PC?
Is there an app for that? or bluetooth? wifi?

Yes it scans and copies but you must manually place a document on the scanner tray each time. Totally fine if you don't copy/scan large documents often.

However, if this had duplex scan/copy you would be able to put a stack of documents in the tray and it would scan/copy the entire stack automatically.
