I never received the email. I took a chance calling Costco travel customer service if they can generate a promo code. The customer rep spoke to a supervisor and was able to generate a code and apply it to the vacation package I booked on 2/4.

Similar situation here, recently booked trip, no code.
Your post motivated me to call Costco Travel. Rep was helpful and got me a code then applied to my booked trip.

I will receive it 2-4 weeks after the trip. They sent me a new itinerary with the updated amount of cash card I will receive
