Pin you have to create in your GV account settings by toggling on "Call to listen" under voicemail settings.

Based on info in this thread as well as info found on google, it is recommended to use google's address if you are porting a GV number and are asked for an address when providing port info for your 'old carrier':

Chase 15% Offer Details (YMMV / Targeted)
NOTE - this deal is YMMV / Targeted. I have several chase credit cards, but the offer only appears on 2 of the cards.
1) Log into chase.com account
2) Accounts tab -> (top left of screen)
3) On the right hand side of the screen you will see a "Chase offers" panel, click it
4) Top right corner there is a "choose account" drop down. If you have multiple chase cards, you will have to cycle through all of them and check the deals below that appear. not all of them will have the Verizon offer (hence YMMV)
5) If you see "Total by Verizon" deal then click on the "+" to add the offer to your card. NOTE - Remember which card you added it to (i.e the last 4 digits of the card)
When checking out on TotalByVerizon.com, use the Chase credit card that you added the offer to, to pay for the phone.
You will get an email notification a few days later, if successful (Subject "Congrats! You've successfully redeemed one of your Chase Offers")

Honestly don't want to call in case say the wrong thing/screw up the port and end up with a $112 brick since it doesn't sound like GV #s are like standard numbers. But thanks for the tip on the address, I do recall an address line but I think I put in my address- didn't realize they wanted GV address.
I don't get why Total wants me to call them- they're saying they couldn't verify some info even though I followed what the wiki here says.
Could be a problem with whatever port info you put in (typo?) -- call and see what error they have to fix
You can update the VM pin in GV settings and use same one again in case that had a typo
I used my name/address/zip along with phone number as account number and gave VM pin -- my GV line was active for few days before porting and my GV account has my name/address:
