Process is to simply active the offer; use the card to add $90 to your Mint wallet; $45 credit will appear on your next Amex statement. You can then use the $90 Mint wallet credit the next time you need to pay for a plan. I've done this multiple times before in the summer, but didn't use it until January (when my annual plan was up for renewal) without a problem.

update: received both $45 credit on my amex cards two days after.

If I were to bring in the line by paying $45 initially for first 3 months and then add $45 to wallet for next 3 months (hoping wallet gives me $45)
Does Amex recognize this and honor the cash back ?
Or how best this offer be used by someone joining new with/without referral?
I used this offer, which appeared on two Hilton Amex cards (mine and my authorized user). I received a "you used your offer" confirmation email for each purchase, which were done days apart. But I only received one $45 credit. I called Amex customer service (in the Philippines) and they insisted I would only get one offer even though it appeared on both my card and my authorized user's card. Turned around and did an online chat…. The online person saw the offer was on both cards… and manually added the second $45 credit.
