Process is to simply active the offer; use the card to add $90 to your Mint wallet; $45 credit will appear on your next Amex statement. You can then use the $90 Mint wallet credit the next time you need to pay for a plan. I've done this multiple times before in the summer, but didn't use it until January (when my annual plan was up for renewal) without a problem.

1. You receive lower priority than T-Mobile and Google Fi customers. For me, the impact is rare, but I do see it from time to time.
2. You can't roam onto AT&T like T-Mobile customers can, so they get better coverage in some areas, though this roaming is becoming more and more rare, so not a huge concern.
Mint will give you at best equal coverage and speed as "real" T-Mobile. Again, this usually isn't a big issue, but it is a consideration. For example if you drive Uber for a living, getting an MVNO that gives priority data (Visible, US Mobile Warp plan, Google FI, etc) is probably a better idea than using Mint or any of the others that use T-Mobile's network (besides Google who somehow got priority, they don't offer that to any other MVNO).

Could be as simple as her needing to get a new SIM card. I replace my e-SIM every year since they're changing things so frequently, and it is free to do. To get the best reception on T-Mobile (or Mint, or any of their MVNOs) you need a phone AND SIM that support the UC and Long Reach 5G bands (and in reality it should be one that supports all of the bands they use, which most phones made since 2020 or so should, but you need to check to be sure).
Almost certain it is device/SIM related, since if anything T-Mobile would have better coverage/speed than Mint, all else being equal.

Edit. …. 20 hours later, I got the email from Amex… "congrats, u used your mint mobile offer".
Edit again…. $45 credit showed up on my Amex account this morning.
