I had similar issues. You just need to hard reset the modem.

Report to FCC, Tmobile has become Ahole company taking advantage of people and wasting their time with incorrect billing and charging even after returning the equipment and asking for cancellation. I did file a report with FCC its easy and you will get a response.

If you previously received a promo on the 5G home internet service ($200/$150 ones from 1-3 months ago) are you eligible for this $300 deal if you sign up for another service? I still have the original home internet service, and could use another service for my other home

Well fun. I submitted the form. Went with date I actually ordered, 12/20, which the email was dated, but the email actually said 12/21, as did a text I got later after activating and submitting for the virtual rebate card. Hopefully since it's still within the promo period it will goes through.
Also confusing that they ask for your phone number in contact info after wanting your Internet line number that it applies to.
Either way I hope it goes though. If they still offer $10/mo "backup" plan I would consider switching to it after the required 2 months is up. Otherwise they probably get the box back.

How is the router/modem powered? A/C adapter to a regular electrical wall outlet? USB and rechargeable battery? Something else?

UPDATE: It is powered by a wall adapter that plugs into a regular electrical wall outlet. (No rechargeable battery.)

Selected date of purchase a day before what the invoice shows even though that's actually when I ordered it. During evening of 20th, but somehow it shows 21st on the invoice.

Promo ends 1/2 I think it showed on the form.

I have the GL-iNet GL-X3000 router/modem and benched the Arcadyan KVD21, but it is $400. So far I think it was worth it. SDX75 modem (vs X62 in the GL-X3000) would be even better, but $700+. Not for everyone. Chester Cheetah is a popular one. Matter of fact you could just skip TMHI altogether add a line to a magenta max+ plan and get better priority than TMHI, but both are against TMHI TOS of course. I have considered it. I do use eero Pro 6 in bridge mode after the GL-X3000.
