What it does is lets you define the resolution, framerate, CPU power, GPU power, etc.. into "profiles" (You can make your own or use the included ones, available for almost every title). It can make the games look and run much better than the original versions.
Sometimes this is for games that were designed around the Quest 1 or 2 and never really updated, so you can crank the FPS to 120hz, or double the resolution, or push more cpu/gpu power to it.. or if you wanted to save battery you can make a profile for that.
You can do all of this without buying the app, but it's an incredibly funky process with sending ADB commands and you won't have the profiles. I don't recommend that. For $6, for even $20, this is a great app.
Set it to load automatically at startup and apply profiles automatically, and you won't even need to mess with it. Updates are done within the app itself, so once you get it installed and running you don't need to mess with it much beyond that.
Very useful even on Quest 3 and 3S.. Keep in mind that this is for games, not videos, so it won't make Youtube or other videos clearer.. it enhances the resolution of UI elements and gameplay elements generated in the headset

Obviously this'll decrease battery life and if you really jack up the settings you'll feel a little more heat coming out of the top vents on the front visor.
