E85 = 15% ethanol dilution.

Ethanol gums up engines - this is why typical gasoline since the 2000s will brick your lawnmower - need to take apart carburetor and clean to get working again.

Using ethanol free gas (usually available at Sheets, Wawa, etc) is best for vehicles, lawnmowers, and everything else. Better MPG as well because gasoline is more energy dense than ethanol.

Ethanol is the reason engines need these cleaners in the first place.

Ethanol is to engines what high fructose corn syrup is to the human body; they both come from corn, heavily subsidized by our tax dollars.

It might. I'd skip it.
But, you may need evaporator cleaner as cheap try to fix the code.
If it will not address the issue then look for O2 sensors, likely to be replaced. You need to check for both before and after O2 sensors via OBD2 to see which one isn't good.
And the worst case CAT converter replacement.
If above are unknown for you then talk to trusted mechanic.

Would this help for a car that's been sitting with old gas? Old car was planning to just try to start it up fill up with fresh gas or drain the tank and the. Start it up again

Ethanol is not great for engines but your last statement about ethanol being the reason for these is a bit misleading.

These cleaners were added to some gasoline brands since the 1950's to prevent engine deposits and required by law since the 1990's to improve fuel economy. While a detergent is required by law, the quality of those detergents vary by brand. "Top Tier" gasoline, defined by vehicle manufacturers to show better deposit prevention, started in early 2000's. Even with ethanol free, you will get deposits and reduced fuel economy. These cleaners are to try and combat that.

it will help. Along with a few tanks of premium fuel. It happens to my 2005 cr-v every winter here in Maine. When i use a bottle of this and fill up twice with premium fuel a few times, it goes away. The code is catalyst efficiency below threshold. Cold temps, and the catalytic converter does not get hot enough to do its job according to the sensors....

Proof: "I purchased a 2007 Chevy Avalanche and almost immediately the check engine light came on. I ran a code scanner on it and said that PO430: Catalyst System Efficiency Below Threshold (Bank 2). I thought for sure I was replacing a Cat or at least 02 sensor. I read up on here to try the Lucas Deep Cleaner first. I had to do emissions on it, so I gave it a try. I immediately threw in a bottle and it cleared the codes and 4 days later, I was shocked when it passed the emissions with no codes. I used Lucas before and will continue putting a bottle in every 4000 miles!

Don't get the cheap 02 sensors either. I went through hell fixing everything after the cheap ones didn't work but assumed they did. From coils to sparks, I was about to do the cat convertor until I was told it was the 02. Since then car works perfect.

like i stated earlier. you dont base on oz per bottle. what you want to look for is the total gallons it can clean up to. they are all labeled the same "concentrate plus"

Definitely not, however check for exhaust leaks. I drove with that code for over a year and thought I needed a new Catalytic converter for sure until mechanic found and fixed a small exhaust leak I didn't even realize I had.
