The rep that helped me scanned my Costco card and told me that it'll be automatically emailed to my Costco account after 90days of the order (in case of returns).

The only thing I don't like about it is that it is noticeably heavier and Pixel OS is really bad (switched out to Niagara). I still have a regular S21 for using an old Galaxy watch and a Samsung tracker since those don't work very well outside Samsung's system.

The phone should be used on tmobile for 40 days, and also fully paid off before it can be unlocked. But you will lose promotional credit if pay off early, so basically two years to unlock.

there is no one time credit option
