I don't follow this premise, he pretends to choke and for some reason the person who saves him feels like they are responsible for his life and send him money? Shouldn't be the other way around? Shouldn't they extort him because they feel like he owes them his whole life?

I'm just thinking if I save someone's life, I might accept whatever gratitude they want to give me in a monthly check, but there's no way in hell I'm going to send them anything. maybe he tries to ask for help with hospital bills or something, but...

So, the way it works is that he is choking they "save" him and experience a euphoria of being a "hero" and to a number of them develop a hero complex of sorts where they would see him as defenseless and lost. So, he asks for money for reasonable things like you mentioned help with dentist or doctor bill, utilities because he lost his job, etc. They made this a movie with Sam Rockwell.
