T-mobile does not publish the end dates of their promotions. No one knows when it will be over, so if you are purchasing a trade-in, make sure you can return it.

I upgraded through T-mobile inside Costco in late August, and received my Costco GC for the upgrade (so 100ish days). I did not have to submit any forms like I had to submit for my previous upgrades; so it's true that T-mobile auto registers you for the benefit. All I did was to set a calendar entry to follow-up in 4 months in case I never received the GC.
The Costco GC was emailed (It's time to claim your Virtual Costco Shop Card) and texted (T-Mobile: As a part of your T-Mobile purchase at Costco...) to the account holder number. It was $100 GC for my upgrade, may changed by now.
