I just chatted with a FiOS rep and was told it's currently 300 Mbps for $49.99/mo with autopay and taxes. With the $15 discount (for 36 months), it will be $34.99/mo with autopay and taxes. This is for 3 years of price lock guarantee. Therefore, the $15/mo discount is already included in the $34.99 pricing, not in addition.

It's an online-only offer. You can't get the deal with a rep.
ETA: to get the add'l $15 you need FIOS mobile plus home.
ETA2: It's unclear. The additional $15 shows up as a bill credit, not a discount. I'm going to try setting up a new account and see if I can get the complete discount
