How do you set these things up? Can I make it work with my car speakers, or does it need to use the speakers (what looks like speakers) built in?

As other poster mentioned, it has a built-in mic, which means you can make handsfree calls as well.
However if your car already has Bluetooth, this doesn't do add anything. If you want to get it for charging port only, better get one that has ports without Bluetooth.

You can also use it as a Bluetooth speaker for audio calls. These types of things are usually only useful on older cars that dont have integrated Bluetooth
