Now you either need to pay monthly a monthly subscription fee to keep this thing on hold (i.e. paying for nothing) or pay an activation fee every time you decide to pause and restart.Edit: looks like Garmin lost enough money with that strategy that they reversed course a few months ago. You can now suspend your service again. That makes this MUCH more attractive IMO.

I haven't done the research on iPhones, but there might be difference in the service. There are several satellite messaging systems with various degrees of coverage, some of which are only good in certain regions or latitudes. There are also different companies that route your emergency communications and dispatch rescue operations. You'd want to compare.
Some of these companies sell things like rescue insurance, so you're not on the hook for search and rescue costs. If you're doing dangerous things that might be a benefit.
Lastly, while it's free now, Apple is supposed to start charging for their service at some point, which might make the comparison more interesting.
If you're occasionally out of cell service while car camping or something a phone might cover it. If you're in any danger of needing real search and rescue, the Garmin (or competitor) might be better.
