If I'm using a down quilt in 20 degree weather it's nice to have a medium density beanie under my hood to plug energy leaks the snug ear design here does that nicely

Cotton is generally a bad idea for working out in freezing weather. A cap like this is designed to be worn under a ski or bicycle helmet where you want a fabric that keeps you dry as well as warm. Wool would also work, but good like finding a good wool cap at this price.
