I was also able to trade in a Galaxy S5, so I don't think that list given earlier is a complete list.

Per the terms, you will get $800 RDC on P10 family for any trade-in in any condition as long as you can get the IMEI number. [reddit.com] So broken, non-functional phones work as long as the IMEI number is physically somewhere on the phone. If the online trade-in cannot find your IMEI, you will have to order through an agent.
Limit for this promotion is the standard 4 per account. T-mobile systems allows you to stack multiple EIPs and RDCs on your lines, as long as the promotions are different [reddit.com]. If you pay off the EIP early, you will lose the corresponding RDC.
