If you're in NYC and having trouble with this offer (as the total keeps coming out to $358.31), use the generic promo code "Welcome" after entering your address and seeing the "Add promo code or gift card" option appear on the lower right portion of the checkout screen.

You'll have to pay shipping ($8.99), but you'll be over the $359 total to trigger the offer. My final total was $368.10 so I'm out of pocket $9.10 after the credit triggers. Not a bad deal.

P.S. - It's ridiculous that shipping is also taxed.

Initially I couldn't get the code to apply. The checkout page said only one discount can apply per order, but i hadn't put in any codes. Then did another browser and was able to apply the code for an extra band. Not sure if the site is recognizing the Chase offer as a discount…

I got confirmation on the credit today! I got an email and it said I was awarded the credit. My order was the $359 program/band +$8 ship +tax. I used promo code and total came out to $36x.xx.

I can't wait to try this device/program. Hopefully it's good. So we'll see if this was a slickdeal or not.

I set mine up but after doing so I realized I could have just sold it. I think the 12 months is linked to the device itself not the account you ordered from as these can be purchased at many retailers and come in a retail box.
