The extras do add up, but some are wroth it. The matte cover is worth the upcharge. Glossy pages are worth it but the 6-color printing really isn't. The metallic text on the cover also doesn't show up well. I haven't done something so important that I'd want to pay the much-higher cost of the lie-flat pages. If you have a lot of pictures that go over both pages, it might be worth it b/c the standard book definitely will hide the very center.
Most of all, unless you're tight on space or making a book for a child, I would skip the 8x8 and 8x11 sizes. I know the other sizes have a higher starting price, but you can get more options with multiple and/or larger photos. I personally like the 11x14 over the 12x12.
If you get the book and there are smears or bad pages, tell them and they'll make it right. It's happened twice to me in dozens of books. I've had to take pictures of the issues to show them, but each time they've sent me another book after reviewing the defects. I've offered to send the bad one back, but they don't want it for obvious reasons. It is nice to know they stand behind their product.
