Somos Verizon FiOS Battle of the Ballers Sweepstakes ~ 9/8/12 (Facebook)

Sweepstakes begins at 9:00:00 a.m. PT on August 11, 2012 and ends at 8:59:59 a.m. PT on September 8, 2012 ("Sweepstakes Period").

Sweepstakes consists of four (4) separate individual Weekly Sweepstakes and one overall Grand Prize drawing. Each Weekly Sweepstakes begins on a Saturday at 9:00:00 a.m. PT and ends at 8:59:59 a.m. PT the following Saturday ("Weekly Entry Period"). The Grand Prize sweepstakes drawing will be conducted from all eligible entries received during the entire Sweepstakes Period.

Only open to permanent legal residents of the fifty (50) states of the U.S. and the District of Columbia ("D.C."), who are physically located and residing therein (excluding Guam, Puerto Rico and all other U.S. and foreign territories and possessions), who are 18 years of age or older as of the date of entry.

After completing and submitting your Entry Information you will then receive one (1) entry into the respective Weekly Sweepstakes and the Grand Prize drawing. As part of the entry process prospective entrants will be offered the opportunity to tweet or re-tweet about the Sweepstakes in order to receive one (1) additional entry ("Bonus Entry") into the respective Weekly Sweepstakes and the Grand Prize drawing.

Limit one (1) Entry and one (1) Bonus Entry per person (based on email address) per day.

The potential winners for each Weekly Sweepstakes will be randomly selected in a series of drawings held on or about the next business day after the end date of the respective Weekly Entry Period ("Weekly Drawing Date") from among all eligible Sweepstakes Entries received for such Weekly Entry Period by the Sweepstakes administrator, an independent compliance organization. Non-winning eligible Weekly Sweepstakes Entries will not be retained and entered into subsequent Weekly Sweepstakes drawings. All eligible Entries will be entered into the Grand Prize Sweepstakes drawing. At the conclusion of the entire Sweepstakes Period one (1) Grand Prize winner will be randomly selected in a drawing held on or about September 14, 2012 from among all eligible Entries received during the entire Sweepstakes Period by the Sweepstakes administrator. The potential Grand Prize Winner and Weekly Sweepstakes # 4 Potential Winners (or, if a resident of a jurisdiction that deems him/her to be a minor, his/her parent or legal guardian) will be notified by phone and/or email at the phone number/email address provided on his/her Sweepstakes Entry within approximately five (5) days of the date his/her name was randomly selected. Weekly Sweepstakes # 1, #2, and #3 Prize winners will be notified of their winning status upon receipt of his/her prize.

Thirty seven (37) Weekly Prizes as described below:

Weekly Sweepstakes # 1 (Ten [10] Weekly Prizes): Each winner will receive one (1) Verizon and WatchESPN branded blanket. ARV of each Weekly Sweepstakes #1 prize is: $15.

Weekly Sweepstakes # 2 (Ten [10] Weekly Prizes): Each winner will receive one (1) Verizon, ESPN Deportes and WatchESPN branded Football Scarf. ARV of each Weekly Sweepstakes #2 prize is: $15.

Weekly Sweepstakes # 3 (Ten [10] Weekly Prizes): Each winner will receive one (1) ESPN Deportes branded Soccer Ball. ARV of each Weekly Sweepstakes #3 prize is: $15.

Weekly Sweepstakes # 4 (Seven [7] Weekly Prizes): Each winner will receive one (1) branded tablet from ESPN and Verizon. ARV of each Weekly Sweepstakes #4 prize is: $575.

One (1) Grand Prize: Grand Prize consists of the following: A trip for Grand Prize Winner and his/her one (1) guest to England to attend a match of the Barclays Premier League ("Match") currently scheduled to take place between November 2012 and April 2013 ("Trip"). Match to be determined by Sponsor in its sole discretion. Trip consists of:
- round-trip, coach-class air transportation for Grand Prize Winner and his/her Guest from a major commercial airport near Grand Prize Winner's residence (as determined by Sponsor in its sole discretion) to a major international commercial airport near the Match (as determined by Sponsor in its sole discretion);
- standard hotel accommodations (single room/double occupancy) for three (3) nights;
- two (2) general admission tickets to the Match;
- round-trip ground transportation between hotel and airport;
- round-trip ground transportation to and from the hotel and the Match; and
- $800 (paid by check) to the Grand Prize Winner (only).

Approximate Retail Value ("ARV") of Grand Prize is: $5,150. IN ORDER TO ACCEPT THE MATCH PORTION OF GRAND PRIZE, WINNER AND GUEST MUST TRAVEL TOGETHER BETWEEN NOVEMBER 2012 AND APRIL 2013 OR ON DATES SPECIFIED BY SPONSOR IN ITS SOLE DISCRETION. IN THE EVENT THAT GRAND PRIZE WINNER CANNOT TRAVEL IN CONJUNCTION WITH THE MATCH, GRAND PRIZE WINNER MAY ELECT TO TRAVEL WITHOUT MATCH PORTION OF GRAND PRIZE BETWEEN NOVEMBER 2012 AND APRIL 2013 HOWEVER NO ADDITIONAL COMPENSATION WILL BE AWARDED.

In the event that the Match does not or cannot take place during Grand Prize Winner's trip as scheduled or at all, for reasons including but not limited to scheduling conflicts, cancellations, postponement, an event of force majeure, or for any other reason, then the remaining components, if any, of the Grand Prize shall constitute full satisfaction of Sponsors' Prize obligation to the Grand Prize Winner, and no other or additional compensation will be awarded. If Match is cancelled or postponed prior to booking, Grand Prize Winner will have one (1) year from date of notification of winning to take trip without the Match portion of the Grand Prize. Grand Prize Winner and Guest must travel on the same itinerary. Grand Prize Trip is subject to air travel and hotel availability, holiday and blackout dates, and all other Grand Prize and travel restrictions. Airline and hotel reservations are subject to availability. Travel and hotel arrangements will be determined by Sponsor in its sole discretion. All Grand Prize elements must be booked and redeemed at the same time, and no changes shall be permitted after confirmation. Once determined, no change, extension, or substitution of Grand Prize Trip dates is permitted, except by Sponsor in its sole discretion. Except as indicated above, no other travel elements (including, without limitation, travel to and from the airport near Grand Prize Winner's residence, insurance, gasoline, meals, gratuities, additional ground transportation, souvenirs, etc.) are included as part of the Grand Prize. Travel tickets are non-refundable and are not valid for business-related or group travel. Tickets are valid only for travel on the airline chosen by Sponsor in its sole discretion. Travel tickets will be issued only to Grand Prize Winner and Guest. Grand Prize Winner is responsible for any tax liability, including applicable international departure taxes or fees, inspection charges, baggage fees, and security charges. Once issued, tickets are non-transferable and may not be reissued once travel has commenced. Open tickets may not be issued and stopovers are not permitted. Grand Prize Winner and Guest must have valid travel documents (e.g., valid government-issued photo identification, passport, visas, or permanent resident card) prior to departure. Sponsors are not responsible for airline seat assignments, special meals, check-in procedures, and baggage limitations. Sponsors shall have no responsibility or liability for cancellations, delays, or any other change by any company or person providing any element of the Grand Prize, and are not responsible or liable for any expenses incurred as a consequence thereof. Grand Prize Winner and Guest specifically release Sponsor and Sweepstakes Entities from any and all claims for loss or damage to baggage or property, or from personal injuries or death, or from loss or delay, arising out of the acts, omissions, or negligence of any independent contractors, such as air carriers, hotels, restaurateurs, transportation providers, medical personnel or other providers of services or facilities. If Grand Prize Winner chooses to take the Trip and/or attend the Match without the allotted Guest, the remaining elements of the Grand Prize shall be awarded and shall constitute full satisfaction of Sponsor's Prize obligation to Grand Prize Winner and no additional compensation will be awarded. If Grand Prize Winner fails to appear in a timely manner for the Match, then Match portion of Grand Prize may be forfeited. Guest, if any, will be required to execute and return a release of liability and, except where prohibited, publicity release (collectively, the "Guest Release"), which must be returned with the Affidavit Release, or Guest portion of Grand Prize will be forfeited. Grand Prize Winner's Guest must be the age of majority in his or her state of residency as of the first date of travel or Grand Prize Winner must be the Guest's parent or legal guardian and must complete and return the Guest Release on behalf of such minor Guest. Grand Prize Winner will receive an Internal Revenue Service Form 1099 for the total ARV of his/her Prize (stated herein).

Total ARV of all prizes is: $9,625. Limit one (1) Weekly Prize per person.
