SWEEPSTAKES RULES:
Sweepstakes begins Saturday, January 12, 2013 and ends Friday, February 15, 2013.
Each winner will be contacted using the phone number provided on the sweepstakes form. If the winner cannot be reached by phone, an email will be sent to the email address provided on the sweepstakes form. The prize will be awarded to another participant if the winner cannot be reached within 14 days of the drawing.
Sweepstakes is open to US citizens and Kentucky residents 18 years and older.
Prizes may be subject to tax. Please speak to your financial advisor to determine if tax is owed.
If you choose 12 personal training sessions: You will receive 12 personal training sessions from Bonita Paul. She works at Bowling Green Parks and Recreation Fitness Facility in Bowling Green, KY. She has a Master's degree in Exercise Science and is certified through the American Council on Exercise.
Commonwealth Health Corporation (CHC) will pay upfront the cost for 12 personal training sessions with Bonita Paul. It will be your responsibility to coordinate session scheduling with her. Ms. Paul is not an employee of The Medical Center or CHC. The Medical Center/CHC is not responsible for the instruction received from Ms. Paul, your results or satisfaction with the personal training sessions. After CHC makes payment to Ms. Paul for the training sessions, further arrangements and questions should be handled between you and Ms. Paul. You should contact your doctor and get his/her approval before you begin any exercise program.
If you choose the iPad mini: We will pay for the purchase and standard shipping of one iPad mini to your residence, limited to Kentucky residence only.
