. . . . . said the doctor to my bride when she had two of my kids in for their wellness visits today. What does that have to do with anything? And the doc didn't let up. My bride kept telling her that she doesn't need to answer that question; so the doc asked it multiple times throughout the visit.

So far, I've only ever been asked on a standard patient questionnaire, never directly by the physician. I treat it just like the menstruation question and many others. I just skip it if I think it's a dumb, N/A or MYOB type question like, in my case alcohol or tobacco use.
I get why they might ask, because for some it might be a health/safety concern that the physician is the only person to ever make them think twice about the potential risks of something, but it's also easy enough either ignore the question on a form, or explain that you don't feel it's relevant to your care. If the physician is insistent, I'd be apt to find a physician more aligned with my sensibilities...
