Sweepstakes begins on September 1, 2011 at 12:00 a.m. Pacific Time ("PT") and ends on November 6, 2011 at 11:59 p.m. PT.
Open only to legal residents of the fifty (50) United States and the District of Columbia who are at least thirteen (13) years old at the time of entry. Entrants who have not reached the age of majority in their state of residence must have permission from their parent or guardian.
How to Enter: There are five (5) ways to enter:

c. Enter from QR Code via Smart Phone: During the Promotion Period, select Invisalign providers may display promotional flyers with a QR code. Scan the QR code with your smart phone and you will be redirected to a mobile web registration form.
d. Enter from Facebook: You can enter the Sweepstakes through the Radio Disney N.B.T. Concert tab on the Invisalign Teen Page on Facebook at www.facebook.co
e. Alternate Mail-in Method: Hand print your name, address, day and evening phone numbers, email address (if any), and date of birth on a 3" x 5" piece of paper and mail it in an envelope with proper postage to "Invisalign Teen Straight Up Celebration Sweepstakes," c/o Wunderman, 2010 Main Street, Suite 400, Irvine, California 92614. Limit: One (1) entry per envelope.
Limit: You may only enter one (1) time by any of the described methods during the Promotion Period.
ONE (1) GRAND PRIZE: A trip to Radio Disney's Holiday concert in Celebration, FL for four (4). The concert is tentatively scheduled for December 10, 2011, but is subject to change. Trip package includes round trip, coach-class air transportation for four (4) from a major airport near winner's home (determined by Sponsor in its sole discretion) to Orlando, FL; three (3) nights' accommodations for four (4) at a hotel determined by Sponsor in its sole discretion (single room, quadruple occupancy) including room sales tax; four (4) tickets to attend Radio Disney's Holiday Concert; four (4) meet and greet passes to meet the performing artists; ground transportation to and from the airport and hotel and to and from the hotel and the concert. Winner must travel on specified dates in December 2011 coincident with the concert date or the prize will be forfeited. Travel must be round trip. Sponsor will determine airline and flight itinerary in its sole discretion. No refund or compensation will be made in the event of the cancellation or delay of any flight. Travel and accommodations are subject to availability. Travel is subject to the terms and conditions set forth in this Sweepstakes, and those set forth by the Sponsor's airline carrier of choice as detailed in the passenger ticket contract. All expenses and incidental travel costs not expressly stated in the package description above, including, but not limited to, meals, incidentals, passenger tariffs or duties, airline fees, surcharges, airport fees, service charges or facility charges, personal charges at lodging, security fees, taxes or other expenses are the sole responsibility of the winner. A winner who is under the age of majority in his/her state of residence, must travel with at least one (1) parent or legal guardian. Siblings or friends of winner under the age of majority in his/her state of residence that are also under the age of majority in their state of residence can only travel if they have the appropriate parental or guardian permissions. Otherwise, travel companions must be eighteen (18) years of age or older (or over the age of majority in their state of residence) as of the date of departure and must travel on same itinerary and at the same time as the winner. Travel companions must execute liability/publicity releases prior to issuance of travel documents. Travel restrictions, conditions and limitations may apply. If in the judgment of Sponsor air travel is not required due to winner's proximity to prize location, ground transportation will be substituted for round trip air travel at Sponsor's sole discretion. Sponsor will not replace any lost, mutilated, or stolen tickets, travel vouchers or certificates. The Sponsor is not responsible if the concert is delayed, postponed or cancelled for any reason, in which event that portion of prize is forfeited in its entirety and no substitution will be provided except as in Sponsor's sole discretion. Radio Disney's Holiday Concert tickets may be subject to issuer's standard rain-check policies and procedures. By accepting these tickets, winner agrees to abide by any terms, conditions and restrictions provided by the ticket. The Sponsor is not responsible if winner does not use ticket on day of event. Approximate Retail Value ("ARV") of Grand Prize: $4,800. Actual value may vary based on airfare fluctuations and distance between departure and destination. Winner will not receive difference between actual and approximate retail value.
