hand print your name, address, day and evening phone numbers, email address, and date of birth on a 3" x 5" piece of paper and mail it in an envelope with proper postage to "NHL Awards Sweepstakes," PO Box 5046, Kalamazoo, MI 49003-5046. One entry per envelope.
Timing: begins on April 16, 2012 at 12:00 a.m. Eastern Time ("ET") and ends on April 30, 2012 at 11:59 p.m. ET
Limit: Each entrant may enter one (1) time per day, regardless of method of entry
Eligibility: open only to legal residents of the fifty (50) United States (including District of Columbia) who are at least twenty-one (21) years old

ONE (1) GRAND PRIZE: A trip for two (2) to Las Vegas, NV to attend the 2012 NHL Awards on June 20, 2012.
