Participants must provide first and last name, a valid email address and complete address including ZIP code. Upon completion of the form, click the "submit" button to send the entry. Entries must be received by 11:59 A.M. (Pacific Time) on September 6, 2012. All entries are subject to Sponsor's Privacy Policy at http://www.callawaygol
ELIGIBILITY: The Sweepstakes is open only to legal residents of the 50 United States (excluding Puerto Rico) and District of Columbia who are 18 years of age or older at time of entry. Proof of age and residency may be required. Employees of Sponsor and its subsidiaries, affiliates, parent company, retail locations, suppliers, distributors, advertising, promotion or any other agencies or entities involved with this Sweepstakes and members of their families (parents, siblings, children, and their respective spouses) and/or those residing in the same household (whether related or not) of each of the above are not eligible to enter or win. Void outside the United States and where prohibited. Sweepstakes also void where restricted in a manner not satisfied by the publication of these terms and conditions on this website. No purchase necessary nor shall a purchase increase odds of winning.
DRAWING: One (1) winner will be selected on or about September 6, 2012 in a random drawing from all eligible entries received during the Promotion Period. The drawing will be conducted by Sponsor, whose decisions will be final and binding in all respects. Actual odds of winning depend upon the total number of eligible entries received during the Promotion Period. The estimated odds of winning are 1:5,000, but will be subject to the final number of entries.
PRIZE: Upon certification by Sponsor as an eligible participant, the winner will receive one (1) each of the following: RAZR Fit Driver, X-Tour Fairway Wood, RAZR X Muscleback Irons (5-9), X Utility Prototype Irons (18, 21 and 24 degree), JAWS Wedges (52 and 60 degree), an Odyssey White Hot XG #1 and one (1) dozen HEX Black Tour Golf Balls, and a RAZR Staff Bag with a total approximate retail value of $2,800 (the "Prize"). No substitution of the Prize or transfer of the Prize to a third party is permitted, except by Sponsor, who may substitute a prize of equal or greater value.
Taxes and duties (where applicable) are the sole responsibility of the winner. All expenses not specified herein, including, but not limited to local, state, or federal taxes on the Prize, are the sole responsibility of the winner. Entry in this Sweepstakes constitutes each entrant's agreement to pay all applicable taxes and comply with all laws.
NOTIFICATION AND COLLECTION OF PRIZE: The potential winner will be notified by email following the drawing on or about September 6, 2012. The potential winner is subject to verification and must meet all eligibility requirements before the Prize will be awarded. In the event that the potential winner does not meet all eligibility requirements, that person may be disqualified and the Prize will be awarded to another randomly selected individual. The potential winner must follow the directions contained in the Prize Acceptance Form provided by Sponsor and email or fax all completed forms, correctly filled out and signed, and provide any additional information that may be required, to Sponsor within three (3) days of notification as a potential winner, including, but not limited to, an Affidavit of Eligibility/Further Release of Liability and Publicity Rights (where legal). Failure to respond within the time specified or other non-compliance with these Official Rules will result in disqualification as a potential prizewinner and void further participation in the Sweepstakes. If a selected entrant cannot be contacted in a timely fashion, or if for any reason a Prize cannot be awarded, another sweepstakes entry will be randomly selected until the Prize has been awarded. Sponsor is not responsible for any failure for any reason whatsoever of the selected entrant to receive notification or for Sponsor to receive a selected entrant's response.

Publicity, Data Collection and Privacy: To the fullest extent allowed by law, by accepting the Prize, the winner consents to the use of his/her name and address and/or likeness for purposes of advertising or trade without additional compensation. By entering the Sweepstakes, all entrants consent to Sponsor's collection, use and disclosure of personal information to any third party for the purposes of administering the Sweepstakes and, unless otherwise specified, for consumer research and marketing purposes. The winner acknowledges that neither Sponsor nor its agents have made or are in any manner responsible or liable for any representation, guarantee or warranty, expressed or implied, in fact or in law, relative to any Prize awarded herein, including, but not limited to, its quality, mechanical condition or fitness for a particular purpose.
Unanticipated Errors and Tampering: Sponsor assumes no responsibility for computer system, hardware, software, or program malfunctions or other errors, failures, delayed computer transmissions or faulty network connections, whether human or technical in nature. Sponsor is not responsible for late, incorrect, lost, illegible, incomplete, stolen, damaged, misdirected entries or claims. Arbitration of Disputes: Any controversy or claim arising out of or relating to this promotion shall be settled by binding arbitration in San Diego, California, in accordance with the procedural rules for commercial disputes set forth in the Comprehensive Arbitration Rules and Procedures of JAMS ("JAMS Rules and Procedures") then prevailing, and judgment upon the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof.
