I had my Cricket account and PIN numbers ready, so the whole process, including credit check with another rep, took about 40 minutes and my iPhone 6s Plus works immediately. The rep also waived the activation fee, gave me my Sprint account number and let me setup an account PIN as well. I also have another phone handy so the rep can talk to me while I had to turn off my phone in order to switch to the Sprint SIM.
I'm in Southern California: downloads in Riverside County, Orange County and high desert (Victorville, Hesperia) varied between 3 and 8 Mbps, uploads betweeen 1 and 3 Mbps. Speeds are not as consistent as Cricket and I experienced more dead spots as well; signal inside buildings is also worse than Cricket (and worse than T-Mobile, was inside movie theater with wife and her T-Mo MVNO phone has full LTE signal and loads web pages fast while my Sprint iPhone shows 3G and can't load a single page).
Thanks to the original op for the deal post and to all posters for the valuable comments: get SIM from Best Buy, call phone when online portal doesn't work, successful port from MVNOs such as Cricket, etc.
Also considered buying the $25 ATT GoPhone and a $2 single day plan at Walmart just to get them lines established so I can quickly be ready to port back out.

The SIM I got is a SPRINT iPhone specific SIM part number SIMGLW436C, which is apparently the same SIM you can get for free from the Apple Store. I don't think you can order this online from Best Buy and I'm not sure what the Best Buy product number is. The rep just got the SIM from a cabinet, it's not displayed on a rack for sale.

I have T-Mobiles $30.00 Walmart plan which is 5GB LTE 100 minutes/text and it gets better everything. Speeds usually 10/4 which is more than fine with me. And there is always signal wherever I go.
I am in a mall in Portland OR and 3 bars 2g cannot load a website. I am glad I manage to get an different account to transfer instead of my T-Mobile number. They're just charging taxes and fees so we shouldn't complain but their CEO said they're doing this hoping we stay. With the service I am getting I might just cancel soon so glad I didn't port my $30 T-Mobile deal number
