What you should have done (and could do in 30 days if you wish to reapply) is to simply apply as a "Sole Proprietor". From that point, pick a category/subcategory that makes sense for you, put minimal levels of income and years in business. I have gotten 3 Chase business cards doing exactly that (2 using my SSN, the 3rd with an EIN), as have a large number of other people - oddly enough, it is easier to get approved following those steps than for legitimate small business owners applying for their businesses.
One thing to know, the buyer protections granted to customers of non-business VISA cards may not come with the business card. You may want to check carefully before using this card for higher risk purchases.

I called for advice, but since I have a small-time business that includes my name I don't have a fictitious business name statement or proof of business location because I use my actual name on all of my bills.
They declined any other proof, so I'm SOL unless I go to the county and get some permits.
I have the Amex platinum business card with a 10k limit so I was hoping this would work. What are the chances I could apply for the other chase ink card with $500 cash reward?

I also had to call for approval (due to credit report freeze i put in place for previous ID theft) and had them expedite the shipping.
I think you have to call the customer service and merge your business cc with your personal cc, so that all cc show up under one login. If not, please let me know how you were able to link your cc together.

Called today to link my business card to my personal accounts. I had to create a new login for the business account and they linked my personal accounts to the new business login. This is because the business account takes precendence. They can't link the business to personal. Only the other way around. At least thats what I was told.

Edit: Nvm I called in and the rep said my 0% is good until April of next year. The letter is stating what my APR would be after or something like that. I still don't know what this arbitration thing is about lol.
