- It will tell you if you qualify, but make sure to verify at checkout that the promotion has been applied (will show as AMEX discount)

- Proceed to checkout and select your Amex Rewards Card as your payment method

I think it's either random or every 3-6 months. On my account, I was able to get one to use in Nov/Dec 2017 and then again got one on the same account in Feb.
