Rebate is valid on purchases made from 07/06/2018 - 08/09/2018.
This model has one review on Newegg and one review on Amazon, both from late July 2018 and they look to be from the same reviewer, so likely not the same model you are running. Amazon has the annoying (and deceptive) practice of aggregating reviews for PSUs from the same manufacturer with different wattages and model numbers, (they do the same thing with hard drives, lumping hard drive reviews together for the same manufacturer and different capacities - i.e. they lump the 1 TB reviews together with the 2 TB reviews, etc.).
Here's a link to the one review on Amazon - if you click on the "All Formats" filter, you'll see it offers an option "Size 450W|Style: BR", which will filter down the reviews to this particular model:
My understanding is that EVGA is a reseller, so PSUs of the same wattage but different model numbers could be substantially different units. This one appears to be relatively new based on the single (same) review on both sites.
That being said, I've had good luck with EVGA PSUs and would probably buy this one if I needed it, it's just one of those "buyer beware" situations if you're basing your decision based on the 2700+ supposed positive reviews on Amazon which really encompass a variety of EVGA models, when there's really only one review.

Great deal, great product. I use it for a server. 1+ year. No complaints.

Both Amazon and eBay could do better at allowing searches cross referenced with exact item viewing ONLY with filtered results
That's actually tempting for a small office mashine that doesn't need a beefy graphics card. Should I buy... Although last time I had rebate from Newegg took them more than six months. I had scratched it out when I finally got it... But still good deal and really tempting...

Why you wanna scratch out of rebates? Whenever a rebate turns out to be a scam, I go after the merchant like Newegg and ask them to give me money back in store credit, if they don't agree, they will get a chargeback for participate in rebate fraud with the manufacturer. I never have to file chargeback because they always agree to issue a refund or store credit for the rebate amount. Fortunately, rebate fraud only happened to me a few times more than 5 years ago, it hasn't happened for the past 5 years for me, maybe due to me buying less than before or maybe due to the rebate companies are starting to get their act together because they are afraid of backlash from merchants from angry customers. Whenever I do a rebate, I make a scan copy of the completed rebate form with the barcode and the stamped and completed envelop just in case, I don't need to make a copy of the receipt because you can grab it from the order history of the merchant website.
