2. How to Redeem Your Offer

2. Trade-in an eligible device.
3. Go to http://www.T-Mobile.com/Promotions within 30 days from date of device purchase.

Most of the questions asked here are either in the promotion posting, or are just stupid. Read before posting.
The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

The reason I ask is the Costco rep says he doesn't need any trade in details now - and asking me to do that online
after that we need to submit online also at tmobile website....
Magenta, T-Mobile One, Essential and their 55+ counterparts !
There are no other plans allowed so I am not sure if it's OK to take risk and start any other plans !
Probably T-Mo learned a lesson last time when they allowed cheaper plans and a huge number of folks started $20/$30 plans and cancelled within 60 days or so.
Again, this is what the Costco guy showed me in their printed material.

I have seen at least 20 posts say the pro max is not inclluded.
Also, another 20 saying if you do this at TM, it will be bill credits. If at costco it is a rebate card.
You must keep the line at least until you get your card - and use it.
