So he didn't inspect?
When you do it online, it asks for your tmobile number, pin and the code for the offer.
Here, you submit a copy of your receipt and of the back of the box (imei/sn)

But the actual trade in, the Costco rep has to initiate it. He puts the details of the phone such as network, gb size, color, model. Rep also answers like 4 questions. Is the screen cracked, does it power on, etc...

Seems like this will cost you more than it should.
the 2 year is fine if you are willing to stay with tmobile for those 2 years.

You mentioned in your calculation that the price of line would be $60 for 90 days, so that's $20 for the additional line per month? If you do it with tmobile for 2 years, you are paying 24 x $20 months for a total of $480. So what is more expensive for you? the $480 or that $60 Costco membership fee? (assuming you are not going to use this line)

No, he didn't even look at the phone. He just said the trade-in is all done through T-mobile. I already did my online submission but didn't see anything about sending in the trade-in. Hopefully I don't have any issues with my rebate.

No, tmobile unlocked phones do not work on cdma. While att phones work
