Stop spreading misleading info the 11 has both GSM and CDMA hardware inside so once unlocked will work on any network that allows unlocked phones to be used

Tmobile won't be able to return your phone if trade in is rejected, you have agreed to receive the trade in value as bill credit. People here have done lots of trade in with T-Mobile, T-Mobile has its reputation for not trying to do bait-and-switch. As long as you follow their t&c, T-Mobile would fix it if anything' went wrong.

I have an iPhone 6s to trade in and there is an issue with home button. Apart from that the iPhone works perfectly fine. No dents or scratches. Will T-Mobile reject the trade-in?

Don't ask people's opinion and gamble on $580 rebate for this type of question. I wouldn't do it but you may take the risk if you don't care much about money.

Costco, on spot. I see there are two blank lines under From on the return label, so I'm just wondering. Since they won't be return the trade in device, I guess I don't have to write anything on the return label.
