The document tells you which items will be accepted for trade. You can NOT do a IP6, or SE, or Samsung, etc.
It really does not matter how much they give you for the trade. The total rebate you will get will total $580.
Yes, this promotion is for people adding T-Mobile new lines. Duh! Think!
Simple math tells you that trading in an XS or XR is probably not going to make sense in most cases. So, if then don't. Duh!
Nobody has asked yet, but yes the phone must be paid for. And yes it must work. And yes it must not be broken. Duh!
And yes, you must have a Costco card to pay for the phone.

We purchased two iphone 11 on 11/4, one iphone 11 on 11/6,
submitted rebates online on 11/13,
shipped trade-in 3 iphones on 11/18,
3 iphones were received on 11/21.
Mine were all denied and I will call next week to see why they list the following reasons:

Pictures of receipts are showing both costco logo and purchase date.
I cannot summit the rebate online because system said the imei number of the purchased phone is not valid. I bought a 11 pro black yesterday at costco.
The Tmobile rep at 877-470-3619 told me that a lot people are having this tech issue, she suggest i keep trying until there tech department fix it. She also suggest i should mail in the trade-in phone asap.

call T-Mo, they have a tool that they can look at the trade-in screen shots and inspection. if so, they will ask you verbally to accept the new offer amount.
this happened to me also. it took me 3 different calls to get a knowledgeable rep to tell me this and resolve it.
