-Simple Choice is NOT eligible. No, your account isn't special. It won't work. You have to change plans
-If you don't walk out of the store w/ the new Iphone in your hand, you will get bill credits. Which means you are stuck for 2 years and the extra you're paying for the line lessens this deal.
-Deal is $500 rebate for 6s phones, $700 for iPhone 7 and beyond. If you have an iPhone 6 or lower, you're getting no rebate. End of story. There are no loopholes or "what ifs".
-Phone screen can't be cracked. Not a "what about a little crack in the corner" comment. It will be rejected. Same goes for "what about a home button not working." Try it, but don't be surprised when it gets rejected. It's your $700 gamble. Stop asking if it will work.
-If your screen is cracked, get it fixed by a 3rd party place before sending in. It's typically less than $100
-Phone can be unlocked after 40 days w/ a request to Tmobile, and then used with any wireless service.
-Can't cancel line until you get your rebate; then go ahead and cancel or port out. Rebate takes 2-3 months to get
-Phone supports dual sim, so put your current ph# on the Esim (just go to a Tmobile store and ask) and the sim card with your new # in the phone. Yes, you can now have 2 numbers on new iPhones. Then you don't risk the phone # not being active while waiting for the rebate.
-If you want a cheap iPhone 7, look on the internet. Don't be lazy. Otherwise try a boost mobile store and read the "instore promo" offers on their website. Some are shady, so ask everything they're charging you for.
-Lastly, don't trust the Costco wireless reps. Hundreds of people here have done this and similar deals. They know better than any rep you will meet, as they've all done these deals.
My local store didn't have the iPhone 11 in stock and I was told I'd still get the GC rebate. After reading all the paperwork, when I got home, this isn't the case. Had to go back to cancel the line and wait for the phone shipment to arrive to go back and return that (which you have to mail yourself with the label provided). 2 weeks later, EIP is still on my account.

EXISTING CUSTOMERS? - This is for new lines. Not lines already with TM, or on another TM account.
VALUE OF TRADE IN - It does not matter. You will get up to $700 in total credit and rebate, or less if your new phone costs less than $700
DAMAGED PHONE - The phone must be free of defects. It can be carrier locked, but not iCloud locked.
Did $580 Deal Last Week - good for you. Now move on.

Went back the next day and when the manager saw I was adding 4 lines to my account he told the rep to make it happen. I even had to show the rep how to do it on his iPad to apply all the discounts
I have bought several phones in the past directly from TM and I didn't need to do this. I could simply sell the device as new and no open box and I would still receive my TM credits.
Read on here someone said we need to open it and make a call to activate it, etc.
Well, I guess another difference the ones I bought from TM earlier (pixel 3a's) weren't new lines, they were on existing lines as another difference.
TLDR: I bought two, may be selling one, wondering if I need to open box or can sell sealed.

The quote you "smugly" referred to is to people that wanted to $580 deal, and then change to $700 deal. You added ANOTHER LINE which of course would work. Congrats to you.
