-Simple Choice is NOT eligible. No, your account isn't special. It won't work. You have to change plans
-If you don't walk out of the store w/ the new Iphone in your hand, you will get bill credits. Which means you are stuck for 2 years and the extra you're paying for the line lessens this deal.
-Deal is $500 rebate for 6s phones, $700 for iPhone 7 and beyond. If you have an iPhone 6 or lower, you're getting no rebate. End of story. There are no loopholes or "what ifs".
-Phone screen can't be cracked. Not a "what about a little crack in the corner" comment. It will be rejected. Same goes for "what about a home button not working." Try it, but don't be surprised when it gets rejected. It's your $700 gamble. Stop asking if it will work.
-If your screen is cracked, get it fixed by a 3rd party place before sending in. It's typically less than $100
-Phone can be unlocked after 40 days w/ a request to Tmobile, and then used with any wireless service.
-Can't cancel line until you get your rebate; then go ahead and cancel or port out. Rebate takes 2-3 months to get
-Phone supports dual sim, so put your current ph# on the Esim (just go to a Tmobile store and ask) and the sim card with your new # in the phone. Yes, you can now have 2 numbers on new iPhones. Then you don't risk the phone # not being active while waiting for the rebate.
-If you want a cheap iPhone 7, look on the internet. Don't be lazy. Otherwise try a boost mobile store and read the "instore promo" offers on their website. Some are shady, so ask everything they're charging you for.
-Lastly, don't trust the Costco wireless reps. Hundreds of people here have done this and similar deals. They know better than any rep you will meet, as they've all done these deals.
My local store didn't have the iPhone 11 in stock and I was told I'd still get the GC rebate. After reading all the paperwork, when I got home, this isn't the case. Had to go back to cancel the line and wait for the phone shipment to arrive to go back and return that (which you have to mail yourself with the label provided). 2 weeks later, EIP is still on my account.

or send the trade-in phone first and then submit the rebate?
Sometime during the day you do the online rebate. Then sometime in the same day you do the mail in.
I would at least do the online thing to make sure it is working. then hand the phone to your mail person.
Or maybe an hour later? Or earlier?

I'm personally going to use the devices I on my account. Somewhat surprised that they allowed 4 lines, but I read in an earlier post that it could be tmobile wanting to show these numbers to merge with sprint quicker. Who knows. Let's see what happens in the future with deals like this from Costco
ETA: I wrote all my info on a Post-It and just gave it to the salesperson. All they asked was if it was an active number. No questions about how long it was active. I was at the store about 1.5 hours after I bought it on eBay and the port went through without any problems.
I also see in wiki that the phone had to be "paid off" to be eligible for trade-in. Is this boost phone considered "paid off"?
Currently on ATT with two lines. In effort to lower monthly cost (and get a new phone while I'm at it), would this Costco deal be the best? If I am just choosing to switch over to TMO, would going thought TMO directly be a better choice?
They actually tried to make me open a new account to get the remaining 3 lines that I wanted. Will probably try another location just in case.

My question is just about the cost of maintaining the 4 lines for a period of 3 months.

Total = $160/month for 4 lines.
