-Simple Choice is NOT eligible. No, your account isn't special. It won't work. You have to change plans
-If you don't walk out of the store w/ the new Iphone in your hand, you will get bill credits. Which means you are stuck for 2 years and the extra you're paying for the line lessens this deal.
-Deal is $500 rebate for 6s phones, $700 for iPhone 7 and beyond. If you have an iPhone 6 or lower, you're getting no rebate. End of story. There are no loopholes or "what ifs".
-Phone screen can't be cracked. Not a "what about a little crack in the corner" comment. It will be rejected. Same goes for "what about a home button not working." Try it, but don't be surprised when it gets rejected. It's your $700 gamble. Stop asking if it will work.
-If your screen is cracked, get it fixed by a 3rd party place before sending in. It's typically less than $100
-Phone can be unlocked after 40 days w/ a request to Tmobile, and then used with any wireless service.
-Can't cancel line until you get your rebate; then go ahead and cancel or port out. Rebate takes 2-3 months to get
-Phone supports dual sim, so put your current ph# on the Esim (just go to a Tmobile store and ask) and the sim card with your new # in the phone. Yes, you can now have 2 numbers on new iPhones. Then you don't risk the phone # not being active while waiting for the rebate.
-If you want a cheap iPhone 7, look on the internet. Don't be lazy. Otherwise try a boost mobile store and read the "instore promo" offers on their website. Some are shady, so ask everything they're charging you for.
-Lastly, don't trust the Costco wireless reps. Hundreds of people here have done this and similar deals. They know better than any rep you will meet, as they've all done these deals.
My local store didn't have the iPhone 11 in stock and I was told I'd still get the GC rebate. After reading all the paperwork, when I got home, this isn't the case. Had to go back to cancel the line and wait for the phone shipment to arrive to go back and return that (which you have to mail yourself with the label provided). 2 weeks later, EIP is still on my account.

My question is, do I need to go back to Costco and argue and try to receive another shipping label for my 2nd iPhone return, or can I do it simply with these 2 blank forms they gave me and filing online for the rebate? Any help is greatly appreciate, sorry if this has been answered before, attempted to read through the 200+ pages but it's difficult to read everything to get the info...

- bought 3 iphones 7 from boost for $106 each on Friday evening with $12 expedited shipping.
- phones shipped on saturday and arrived today - kudos to boost and fedex
- went to local costco and verified they have phones in stock (only had 3 Iphones 11 64 gigs, not exactly what I was looking for but not too bad either)
- bought costco membership and added it to my app
- rep could not add a line and had to spend an hour with t-mobile support which was less than helpful
- rep admitted that she couldn't promise the rebate to work if line was added via phone rather than their kiosk system
- i suggested t-mobile rep to F*** OFF as they weren't helpful and asked kiosk rep to try again which worked then.
- came back home and submitted 3 rebates without any errors
So all in all i left with 3 purple Iphones 11 64gb and ~$2000 less in my pocket. Submitted rebates without any issues online and hopefully they will work.
The downside is that I was on t-mobile one with 20% hook up code which and their system would only allow magenta so I had to switch and my hook up code was lost, I figured that I have some benefits from my employer which I am trying to utilize now to save a few bucks on my current bill.
What I can pinpoint is that I was there in CT during winter storm around 5pm and the line was like 10 people all asking for t-mobile iphone 11 deal. They also had to answer a phone call about this deal every 5-10 minutes. The deal seem to be extremely popular so no surprise there is no stock.

May I ask what is the phone color you bought? I read initially buyers were getting error for mostly purple ones. I have not submit mine yet, so just curious.

On that 20% off, I would suggest doing it later when you get your bill and ask TMO if you could add it back. In my last job, that is what I did with Sprint. ymmv.

Found a purple one with 64gb but not really what you want to walk into business meetings with. When I call and ask about stock order arrivals the employees had not a clue, some even rude they are too busy to answer if they have. Tmobile and Costco is loosing business because of lack of inventory preparation. Wasted 4.25 hours in store and another 1+ hours calling each store for morning stock arrivals.
Deal is basically dead with any phone stock, unless you want to be on the hook for 24 months of small reimbursements.
My gut tells me that you need 2 forms, because my assumption is that Assurant scans the label to help it quickly identify and link your trade-in phone with your account. If you send in 2 phones in 1 box under 1 shipping label, you could run into the issue of where they misplace your second phone. Also, there's also the more concerning matter that the kiosk rep didn't even process the rebate on one of your lines, in which case you'll definitely have issues. So I would go back and hopefully you'll find a rep who knows what they're doing.
