-Simple Choice is NOT eligible. No, your account isn't special. It won't work. You have to change plans
-If you don't walk out of the store w/ the new Iphone in your hand, you will get bill credits. Which means you are stuck for 2 years and the extra you're paying for the line lessens this deal.
-Deal is $500 rebate for 6s phones, $700 for iPhone 7 and beyond. If you have an iPhone 6 or lower, you're getting no rebate. End of story. There are no loopholes or "what ifs".
-Phone screen can't be cracked. Not a "what about a little crack in the corner" comment. It will be rejected. Same goes for "what about a home button not working." Try it, but don't be surprised when it gets rejected. It's your $700 gamble. Stop asking if it will work.
-If your screen is cracked, get it fixed by a 3rd party place before sending in. It's typically less than $100
-Phone can be unlocked after 40 days w/ a request to Tmobile, and then used with any wireless service.
-Can't cancel line until you get your rebate; then go ahead and cancel or port out. Rebate takes 2-3 months to get
-Phone supports dual sim, so put your current ph# on the Esim (just go to a Tmobile store and ask) and the sim card with your new # in the phone. Yes, you can now have 2 numbers on new iPhones. Then you don't risk the phone # not being active while waiting for the rebate.
-If you want a cheap iPhone 7, look on the internet. Don't be lazy. Otherwise try a boost mobile store and read the "instore promo" offers on their website. Some are shady, so ask everything they're charging you for.
-Lastly, don't trust the Costco wireless reps. Hundreds of people here have done this and similar deals. They know better than any rep you will meet, as they've all done these deals.
My local store didn't have the iPhone 11 in stock and I was told I'd still get the GC rebate. After reading all the paperwork, when I got home, this isn't the case. Had to go back to cancel the line and wait for the phone shipment to arrive to go back and return that (which you have to mail yourself with the label provided). 2 weeks later, EIP is still on my account.

No reason to get gray hair over a delayed tracking. It is the holiday season afterall. Many folks have seen it sit for days and days.
There is no "order" of how to do this stuff. Only reason I would do the online first is to make sure it is all working okay. Nobody has any idea if you put the old in the box for shipping before or after you did the online - until it shows up.
Go online. fill out the stuff.
Then send in your phone.

Transfer to your bank directly from the rebate card website. There is no fee to do this.
Log in to the rebate card webpage and click the 'Access $' tab, then transfer. The 'transfer' button is also on the 'Dashboard' tab.
You can only have your bank info attached to 1 rebate card at any same time. So, once you initiate your bank transfer, remove your bank info.
Now log on to the next rebate card and repeat the process until you have all your cards drained.

USPS showed my trade-in phone shipment tracking the evening I took it to them.
Whether the order matters or not, I don't know. Kiosk person was emphatic that I ship first and enter rebate online after, even noting it was different from last year, so I did it that way.
I don't think it matters though.

FWIW, it was delivered on the 5th late in the day, and I received a text on Saturday the 7th stating that T-Mobile had applied my device trade in credit to my account.
