-Simple Choice is NOT eligible. No, your account isn't special. It won't work. You have to change plans
-If you don't walk out of the store w/ the new Iphone in your hand, you will get bill credits. Which means you are stuck for 2 years and the extra you're paying for the line lessens this deal.
-Deal is $500 rebate for 6s phones, $700 for iPhone 7 and beyond. If you have an iPhone 6 or lower, you're getting no rebate. End of story. There are no loopholes or "what ifs".
-Phone screen can't be cracked. Not a "what about a little crack in the corner" comment. It will be rejected. Same goes for "what about a home button not working." Try it, but don't be surprised when it gets rejected. It's your $700 gamble. Stop asking if it will work.
-If your screen is cracked, get it fixed by a 3rd party place before sending in. It's typically less than $100
-Phone can be unlocked after 40 days w/ a request to Tmobile, and then used with any wireless service.
-Can't cancel line until you get your rebate; then go ahead and cancel or port out. Rebate takes 2-3 months to get
-Phone supports dual sim, so put your current ph# on the Esim (just go to a Tmobile store and ask) and the sim card with your new # in the phone. Yes, you can now have 2 numbers on new iPhones. Then you don't risk the phone # not being active while waiting for the rebate.
-If you want a cheap iPhone 7, look on the internet. Don't be lazy. Otherwise try a boost mobile store and read the "instore promo" offers on their website. Some are shady, so ask everything they're charging you for.
-Lastly, don't trust the Costco wireless reps. Hundreds of people here have done this and similar deals. They know better than any rep you will meet, as they've all done these deals.
My local store didn't have the iPhone 11 in stock and I was told I'd still get the GC rebate. After reading all the paperwork, when I got home, this isn't the case. Had to go back to cancel the line and wait for the phone shipment to arrive to go back and return that (which you have to mail yourself with the label provided). 2 weeks later, EIP is still on my account.

Bought iPhone Purple for my wife on 11/25, ran into ineligible IMEI issue while trying to submit rebate online. After multiple calls with the promotion team, I escalated to T-Mobile CEO via Twitter. I was the one who initiated that Twitter escalation and thanks everyone who supported that by responding to it. Due to that escalation, T-Mobile did an overnight fix and we're able to submit rebates around 11/26-11/27. That's when I submitted mine as well.
Sent trade in and was received on 11/30. Called in on 12/11 and they asked to accept $59 (instead of $68 keyed in by Costco kiosk guy) credit trade in value. Accepted and received $59 credit on 12/12.
Rebates status changed to denied on 12/27, with same ineligible trade in found error that most also saw. Called into promotion line and fought hard. They said wait for 01/04 because they can't override the system until it's 30 days after the promotion end date (12/04). In parallel, I initiated twitter direct messages with T-Mobile, using the previous escalation thread.
Yesterday (01/03), Twitter direct message guy said that he was able to put in an override and I should get paid in 7-10 days. I saw the status changed to Approved and then to received yesterday evening. However the status changed to approved again this morning and I just received the rebate card for $629.99 (iPhone price before tax).
All is well that ends well I guess. Rebates are always risky...

Status - Received forever.
Called in Promotions center, rep told me there is an error at the backend and she submitted 2 new rebates. They showed up as approved instantly and then in received after 2 hours and as Denied next day with reason "Device already used for an existing submission."
And original rebates submitted by me are still in Received state.
If your rebates get denied, and they open a new rebate - It works fine and you get rebate card.
But if your original rebate is in Received state, they are unable to do anything and asking one more week everytime.
If something changes for you or anyone in same boat , please update.

12/5 - Submit online rebate forms
12/16 - Scheduled USPS pick up for in use Verizon trade-in phones
12/20 - USPS delivered the trade-ins
12/22 - Device trade-in credits applied to account
1/5 - Received rebate card texts and initiated ACH. Trade-in credits+ rebates total = $1400
Now waiting for:
- ACH to settle
- Unlock iPhones. Online Rep said I have to wait 40 days and he will do it on 1/17 for both phones.

The only unfortunate thing for me in this whole deal was that unlike most others, I did not get the full $700 back and got only $679.99 that I paid for iphone 11, 128 GB. I am okay losing $20, because after denial of my rebate on 1/1, at one point, I was concerned about losing the entire money.
My Total Out of pocket for this deal was
$679.99 (iphone11, 128GB) + $52.69 (Tax) + $138.45 (MetroPCS iphone 7) + $70 (1st Month Bill) + $70 (2nd month bill) - $54 (Device Credit) - $625.99 (Rebate) -$46 (what I would have paid for Ultra Mobile for 2 months) = $285.14
Very good deal I'd say for an iPhone 11, 128GB.
