Glad I did. $300 is better than $100 and I can still port out in a few months. No contracts on the Kickstarter plan. They automatically waived the activation fee by doing it online as well, but after looking at my confirmation email it says there will be a one-time credit of $30 within 3 invoices however that plays out. My first bill will be charged 30 days from today.
I was concerned with the Amazon reviews saying they were sent 4 of the same SIMs, but it worked flawlessly. Over chat the rep told me which one to use. I also asked if I could double down on both the $300 and $100 Amazon SIM promotion, but was a no go. Worth a shot anyway.

I wouldn't worry about the Missing items, that just means Sprint hasn't provided the rebate processor with the info yet. Especially the Missing Channel Code. See the attachment of post #6932 by kenpet. That was a reward that was paid out despite having the Missing Channel Code. I suspect that the Channel Code is mainly about through which channel the order was made (online, by phone, in store, etc).

Should I be worried?
All my lines were BOYD if that makes any difference. Credit check account had 1 GV number and no credit check account had 3 GV numbers.
