The $40 Costco Gift Card will be Mailed to you within 4-6 weeks.
You can enroll in auto-renewal at the same time you complete your online redemption if your state requires you pay sales tax. Tax will be listed under Order Summary on the membership checkout page.
If Tax is listed as $0.00 in your Order Summary, you will need to enroll in auto-renewal after redeeming your membership. You may do so under the Membership Details section on your Costco confirmation page or from your Costco Account Details page.

edit: Groupon has confirmed in this thread that both the $40 off $250 online code and the $40 Shop Card expire at the end of January.
Also, the Groupon offer itself is available until August 31st.
Remember to sign up at BEGINNING of the month as membership always expires at end of month, 13 for 12.

EDIT: To clarify, to qualify for this deal, you need to purchase the membership by using a Visa credit or debit card.
Costco is good for in store, not so much with shipping.
So do I qualify for this deal ? Do I need to use the voucher against my old account or should it be a new account signup ?

I put in my Visa card and it did not give any option to opt into auto-renew.
I then put in the $60 card code and PIN and bought the membership.
I then went into the account and saw auto-renew was not active, so I entered my Visa number and activated it. Hope this does not screw up the promotions that are supposed to be attached to my account.Guess I have to wait 4-6 weeks to find out.
Sam's is getting all my business with their In store pick up, and now even easier with Curbside where they load it into your vehicle.
I hope Costco would offer pick up services soon.

If you didn't sign up for auto-renewal with a Visa card, or if you don't receive your benefits by the end of that time frame, you'll need to contact Costco directly for assistance. You can reach Costco at [email protected] and provide your name, Costco membership number, and mailing address, as well as letting them know the situation.
