No, it can be on any AT&T Prepaid plan.

True. In the past AT&T was one of the easiest to unlock through eBay but I'm not sure if this is still the case as it's been over a year since I went that route. In the past for under $10 you could get an iPhone unlocked and I've done it several times.

Yes, but you may have to call ATT customer service and tell them you got a new phone. I did that once and had no issues. Did it another time and had to call 611 and told them I got a new phone and was up and running in a few minutes.

If you're doing the prepaid thing like myself until the new iPhone launches in a few weeks....
I paid 131.50 for the New iPhone SE on AT&T prepaid with of month of service!
Basically the iPhone SE is $49 bucks!
Must sign up in store and auto pay!
But worth it.... the guy at Walmart was shocked!
