Anyways, I was able to finally activate it after 2 different chats and 2 different phone calls. They had me insert the original SIM back into the phone, and transferred the minutes/texts/data onto the phone and all is working. Unfortunately, they only credited me with 1200MB of data. (The rep actually said "Oh, you got the 1200minutes, 1200texts, 1200MB data plan for $60" and I said "No, I got 1200/1200/3GB for $30" and he goes "Nooo... it's 1200MB") Whatever, it's for my 76 year old father, he isn't going to use that much data and the phone works, so.... ?
At one time some of these (mostly the no specific network choice) did come with separate activation cards that had a pin but they stopped doing that months ago & replaced with preloaded sims for each network.
So on my 2nd chat, I asked if the SIM card was loaded with the prepaid. He asked for the IMEI # of the phone I was trying to use, then the 3GB/1200MB chat from above happened, then he asked "Would you like me to activate those minutes onto that phone?" Um.. yeah!! All is well now. Hopefully this can help somebody else out, or I look like an idiot

1. I am using a BYOD unlocked iPhone.
2. Return was easy: a return shipping label was provided right after filing a refund claim.
